RELEASE AND WAIVER OF LIABILITY

● I, the undersigned, acknowledge that I have voluntarily agreed to participate in fitness and exercise instruction (“Instruction”) offered by Trina Altman LLC.
● I AM AWARE THAT THE INSTRUCTION MAY INVOLVE STRENUOUS BODY MOVEMENTS THAT CARRY A RISK OF INJURY. I AM VOLUNTARILY PARTICIPATING IN THE INSTRUCTION WITH KNOWLEDGE OF THE PHYSICAL RISKS INVOLVED, AND AGREE TO ASSUME ANY AND ALL RISKS OF INJURY, WHETHER THOSE RISKS ARE KNOWN OR UNKNOWN.
● In consideration of participating in the Instruction, I forever release Trina Altman LLC and its respective directors, officers, employees, volunteers, agents, contractors, and representatives (collectively “Releasees”) from, and promise not to sue on, any and all actions, claims, or demands that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives now have, or may have in the future, for injury, death, or property damage, related to (i) my participation in the Instruction, (ii) the negligence or other acts, whether directly connected to the Instruction or not, and however caused, by any Releasee, or (iii) the condition of the premises where the Instruction occurs, whether or not I am then participating in the Instruction. I also agree that I, my assignees, heirs, distributees, guardians, next of kin, spouse and legal representatives will not make a claim against, sue, or attach the property of any Releasee in connection with any of the matters covered by the foregoing release.

I AM 18 YEARS OR OLDER. I UNDERSTAND THE LEGAL CONSEQUENCES OF SIGNING THIS DOCUMENT, INCLUDING (A) RELEASING TRINA ALTMAN LLC FROM ALL LIABILITY, (B) PROMISING NOT TO SUE TRINA ALTMAN LLC, (C) AND ASSUMING ALL RISKS OF PARTICIPATING IN THE INSTRUCTION.
